[PATCH] perfmon cleanup patch

This patch includes:
        - nasty macro bug fix for  PMC_IS_MONITOR()
        - pfm_stats is now per CPU
        - pmu_owners[] is not cache line aligned to avoid false sharing
        - cleanup in /proc/perfmon
        - rewritten pfm_write_pmcs(), pfm_write_pmds() with correct semantics
        - all copy_*user() are gone
        - we do not walk the tasklist in UP mode for system-wide: now
          unified with SMP
