misc hcd cleanup

  
This is minor cleanup; pulls #includes out of files
that aren't intended to compile by themselves.

ehci bandwidth recording
  
Here's a minor update to the EHCI interrupt scheduler,
recording the bandwidth used by an URB for usbfs.
