[PATCH] loop needs MODULE_ALIAS_BLOCK

There are two issues here:

- absense of a MODULE_ALIAS_BLOCK in loop.c

- mismatch between the patterns used in the MODULE_ALIAS_BLOCK define and
  the modprobe invokation in request_module.
