Pull USB/PHY fixes from Greg KH:
 "Here are some USB and PHY driver fixes for 4.8-rc5

  Nothing major, lots of little fixes for reported bugs, and a build fix
  for a missing .h file that the phy drivers needed.  All of these have
  been in linux-next for a while with no reported issues"
