[PATCH] IB/mthca: mem-free work request posting

Implement posting send and receive work requests for mem-free mode.
Also tidy up a few things in send/receive posting for Tavor mode (fix
smp_wmb()s that should really be just wmb()s, annotate tests in the
fast path with likely()/unlikely()).
