replaced with var tags for standard headers and footers.  While this
is sometimes very useful, it is sometimes disastrous.  This feature is now
enabled with the -D option.

Handling of files with a '.dtml' extension was broken. These are
now handled correctly. Files with '.dtml' suffixes now get uploaded as
methods.

A workaround for old sites that had a bug in Document (aka DTMLMethod)
upload has been disabled. This workaround is now enabled when the -9
option is used.

Added logic to try and figure out a path to ZPublisher if one was not
provided.

Fixed a bug in misshandling binary files on windows.

Added an option, -I, to automatically add an index_html method that
redirects to index.html or index.htm.

when JB took over PCGI the header length changed to 10 (from 9) and
there was a buffer with a hard-coded length of 10. The buffer is now
sized to HDRLEN + 1.

  (in order to enable REMOTE_USER mode) also opens the monitor to any
  user connecting through localhost.

replaced it with URL1 to make netscape servers happy.

that is used in the windows binary distribution will always be the exact
same file as the current z2.py (but in windows crlf format, because the
WISE installer won't correctly do the path replacement otherwise). The
bit of hackery does not affect platforms other than windows.

Synched with latest medusa sources from CVS. The main changes are the structure of asyncore.socket_map which now maps fds to channels which should provide a slight performance improvement.

Work around some weird Netscape bug that confuses the help button onSubmit method with the shutdown onSubmit method.
