[AGPGART] intel agp init cleanups.

(1) Kill the _setup functions - most of it can be nice, static
    structs - the few remainders are handled better elsewhere
(2) _one_ big switch in ->probe assigning these tables.

Almost 200 lOC gone and it's even readable :)
