[PATCH] rmap 13 include/asm deletions

Delete include/asm*/rmap.h
Delete pte_addr_t typedef from include/asm*/pgtable.h
Delete KM_PTE2 from subset of include/asm*/kmap_types.h
Beware when 4G/4G returns to -mm: i386 may need KM_FILLER for 8K stack.
