libcxgb,iw_cxgb4,cxgbit: add cxgb_get_4tuple()

Add cxgb_get_4tuple() in libcxgb_cm.c to remove
it's duplicate definitions from cxgb4/cm.c and
cxgbit/cxgbit_cm.c.
