IB/rdmavt: Add missing spin_lock_init call for rdi->n_cqs_lock

commit a6580f43 upstream.

This fixes the following warning with PROV_LOCKING enabled kernel:

the code is fine but needs lockdep annotation.
turning off the locking correctness validator.
