Fix issues mentioned but not closed for JIRA

The `ReferenceExtractor` would return an array of `ExternalIssue`
objects, and then perform `Array#-` to remove the issues
closed. `ExternalIssue`s had `==` defined, but not `hash` or `eql?`,
which are used by `Array#-`.
